We are looking for an ambitious, enthusiastic, motivated individual who will be required to deputise for the Community Assessment Unit lead in helping the team in taking the next steps in developing community services. If you have a strong interest in reablement, working with patients' strengths and promoting their independence to enable them to live safely in the community this could be for you?
We are looking to recruit a Band 6 RGN to work as a Charge Nurse/Sister in our brand new Community Assessment Unit due to open in early 2021. Working with a full team of therapy staff/nurses/generic support workers/speech and language and dietetics/mental health workers, we aim to provide patient focused care utilising a strength-based reablement approach.
The CRT (Community React Team) works in collaboration with NWAS (North West Ambulance Service), primary care and wider health economy in order to reduce hospital admissions.
The Community Assessment Unit (CAU) will consist of 21 beds including 9 single rooms. Its primary function will be to operate as an older persons assessment unit, assessing patients for suitability for step up/step down care and safe discharge into the community plus there will be access to diagnostics for community via the 6 assessment chairs.
The role will require you to work across all areas in the Trust, which includes those that are designated as positive, asymptomatic and negative COVID wards.
An Enhanced DBS with Adults Barred List check will be obtained for the successful candidate during pre-employment checks.
Based in the Community division so you will need to be able to travel to Ashton, Wigan and Leigh.
